Oasis Recovery accepts adults committed to sobriety who are willing to participate in our structured program, including drug and alcohol screening, mandatory house meetings, and four weekly recovery meetings.
How the application works
- Initial inquiry. Call our operations team or submit the intake questionnaire below. We will follow up within one business day.
- Pre-admission screening. A brief conversation about your recovery goals, sobriety history, current treatment, and any clinical or financial considerations.
- Intake. Approved applicants receive a secure invitation link to complete the full intake packet, including the Sober Living House Agreement and Resident Handbook acknowledgment.
- Move-in. Upon arrival, residents complete a drug and alcohol screening and receive a tour of the house, room assignment, chore list, and access to our resident app.

What to expect at intake
If you are admitted, you will receive a secure link to complete the full intake packet on our resident app, Anchor. The intake includes:
- Contact information and emergency contact
- Recovery history, substances of choice, sobriety date, mental and physical health history, current medications, treatment and sober-living history
- Sponsor and clinician contact information (if applicable)
- Legal status (probation, parole, or pending matters, if applicable)
- Signed Sober Living House Agreement and Resident Handbook acknowledgment
- SMS consent and Release of Information forms as needed
